Want to see first-hand how the vanguards are delivering new models of care?

Health and care leaders will have the opportunity to see first-hand how new models of care are being delivered by the vanguards and others.
Led by those involved in leading and developing the vanguards, the visits will involve a tour of local services, allowing health leaders to see change in action for themselves. They will also include an overview of the local health and care economy and opportunities to meet local leaders to share experiences of leading transformation programmes.
Upcoming vanguard visits will take place:

Monday 26 February, with Greater Manchester Health and Social Care Partnership. Join senior leaders from across partnership organisation to learn the unique transformation taking place in Greater Manchester as a result of devolution. This visit provides the opportunity to understand more about Greater Manchester's whole systems approach by showcasing how integrated and place-based partnership working is happening in practice. Gain practical insight into how local partners – particularly the NHS and local authorities – have overcome the barriers that have historically precluded partnership working.
Previous visits have included:
- Tuesday 31 October, with North East Hampshire ‘Happy, Healthy, at home’ vanguard, including visits to specialist health and social care teams supporting individuals with complex health and care needs. Read our blog here.
- Wednesday 15 November, with Nottingham and Nottinghamshire Sustainability and Transformation Partnership (STP) which included presentations from a number of local vanguards as well as discussion with senior leaders from the STP. Read our blog here.
- Thursday 11 January, with Connecting Care Vanguards in Wakefield. This visit included a visit to an 'Independent Living Scheme' which is part of their Enhanced Health in Care Home Vanguard, alongside shared learnings from their Multispecialty Community Provider Vanguard.
- Thursday 8 February, with Tower Hamlets Together. This visit included discussions with senior leaders from partnership organisations across Tower Hamlets to hear about their work to develop primary care networks and move towards longer term primary care sustainability, as well as developing accountable care arrangements in the borough and across the STP.
Further visits will be added to the schedule in the future.
The visits form part of a national programme to share the learning from new models of care across the health and care sector. The programme is being delivered by the NHS Confederation, NHS Clinical Commissioners, NHS Providers and the Local Government Association.
