  • Patient action tracker – an in-house developed system to help manage discharge and flow.
  • Support, transition and recovery team in mental health and children in care and care leavers health team.
  • Frailty model – integrated care of older people teams between acute and community services. Home treatment team led by community physicians/geriatricians.
  • Hampshire 20/20 leadership development programme – cross organisational, cross professional, cross hierarchical leadership programme.
  • GP link speciality programme connecting GPs and hospital colleagues.
  • Frailty response car – run by ambulance, community, and hospital colleagues.
  • Morecambe Bay respiratory network: A clinically led and integrated network that takes a multi-disciplinary team approach to improved access to specialist respiratory advice and clinical guidance. Has reduced outpatient referrals, improved education for primary, community and secondary care clinicians, increased confidence and competence.
