Provider collaboratives have played a significant role in contributing to the reduction of deep-rooted health inequalities across their geographies. As with reducing variation, having access to and understanding the data at a system level is key. Provider collaboratives are using population health data and public health expertise to ensure equitable access and provision in the services they design and deliver.
