Supporting people and communities to stay well

We highlight how the vanguards have shaped local health and care services to help people stay well for longer, avoid longer hospital stays and improve people’s access to services. 


Learning from the vanguards: supporting people and communities to stay well explores how the vanguards have sought to design health and care services around the needs of people who use them, focusing on the outcomes that matter to people and tailoring care to their needs and goals. It also explores how the vanguards have adopted community- and asset-based approaches to consider the broadest possible influences on health and care.


Wellbeing Erewash vanguard

Hannah Leaton, vanguard programme lead for Wellbeing at NHS Erewash CCG discusses her experiences at the helm of Wellbeing Erewash vanguard, as part of our series of resources to help share the learning from the new care models. 

In conversation with Matthew Mcnair-Smith, policy lead at the NHS Confederation, Hannah explores Wellbeing Erewash vanguard's 'time swap' programme and how it helps people and communities to stay well.

Hannah also shares some of the challenges the vanguard has faced, including bringing together a wide range of partner organisations and building a shared vision, particularly across the NHS and local government. She also discusses how the vanguard overcame recruitment issues.


Better Care Together vanguard

Heather Tierney-Moore, chief executive of Lancashire Care NHS Foundation Trust, and Jackie Daniel, chief executive of University Hospitals Morecambe Bay NHS Trust discuss the Better Care Together vanguard, one of nine integrated primary and acute care systems being piloted in England.

The conversation, led by Saffron Cordery from NHS Providers, covers Heather and Jackie’s views on the motivations and challenges involved in establishing a new approach to delivering care, and the changes required to bring about real improvements for their communities.

A unique offering for patients with highly complex needs
Many over-60s have multiple conditions and highly complex health needs. To better support them, the Fylde Coast local health economy has developed an innovative new care models that takes a more holistic appraoch to their care and helps to keep them out of hospital. Senior responsible officer for the vanguard, Peter Tinson, introduces the initiative.
