Learning from the vanguards: improving the experiences of people who use services looks at what the vanguards have been doing to improve the way people experience and interact with health and care services, and shares the lessons that other organisations and partnerships can take from the vanguards’ experiences.


Connecting Care Wakefield

Lesley Carver, senior project manager and Zoe Richardson, project officer from Connecting Care Wakefield discuss how the Enhanced Health in Care Homes vanguard is improving the experience of people accessing services in the area.

In conversation with Francesca Reville, senior programme manager at the NHS Confederation, Lesley and Zoe discuss some of the initiatives that are improving the way patients access care including the 'Red Bag' scheme, which improves access to patient records and ensures people only have to ‘tell their story once’ throughout their care journey. They also highlight the 'Portrait of a Life' scheme, developed in partnership with the local mental health provider.

Lesley and Zoe share how the initiatives have impacted patients and the findings of a year-long evaluation. They also share their advice for colleagues planning similar schemes.
