On 29 June 2023 the Parliamentary and Health Service Ombudsman published a report on patient safety, 'Broken trust: making patient safety more than just a promise'.

The report sets out the key findings from a review of 22 of the most serious cases investigated by PHSO, and makes several recommendations to improve responses to patient safety incidents and make patient safety a more consistent priority.
