The new framework aims to provide guidance to local healthcare systems on how to conduct “strategic, preventative, collaborative, fair and just, credible and people focused” investigations into any safety breaches. This follows the introductory version of the PSIRF, published in 2020, which was piloted with 17 early adopter organisations, whose feedback helped inform this final version.
