Want to see first-hand how the vanguards are delivering new models of care?

Health and care leaders will have the opportunity to see first-hand how vanguards are delivering new models of care and implementing innovative transformation programmes as part of an exciting series of vanguard site visits.
Led by those involved in leading and developing the vanguards, the visits will involve a tour of local services, allowing health leaders to see change in action for themselves. They will also include an overview of the local health and care economy and opportunities to meet local leaders to share experiences of leading transformation programmes.
Upcoming vanguard visits will take place:
- Thursday 11 January 2018, with Connecting Care Vanguards in Wakefield. This visit will include a visit to an 'Independent Living Scheme' which is part of their Enhanced Health in Care Home Vanguard, alongside sharing learnings from their Multispecialty Community Provider Vanguard. Senior leaders from across partnership organisations who are part of the Connecting Care programme will be on hand to share their experiences and their lessons from the programme.
Previous visits have included:
- Tuesday 31 October, with North East Hampshire ‘Happy, Healthy, at home’ vanguard, including visits to specialist health and social care teams supporting individuals with complex health and care needs.
- Wednesday 15 November, with Nottingham and Nottinghamshire Sustainability and Transformation Partnership (STP) which included presentations from a number of local vanguards as well as discussion with senior leaders from the STP.
Further visits will be added to the schedule in the future.
The visits form part of a national programme to share the learning from the vanguards across the health and care sector. The programme is being delivered by the NHS Confederation, NHS Clinical Commissioners, NHS Providers and the Local Government Association.
